Here is a proposed itinerary: (would only require 8-9 days of vacation time!!)
Nov 14: Fly USA-Bali
Nov 16: Fly Bali-Ujung Padang (1 overnite)
Nov 17: Fly Ujung Padang -Sorong, and aboard the Seahorse

Nov 17-27: Diving, diving, diving, in Raja Ampat

Nov 27: Fly Sorong- -Bali (and home if you want)
Nov 27-30: Free days in Bali or UBUD (tbd)
Nov 30: Fly Bali-USA, arrive USA in the evening

Package:
10 nights on board the Seahorse-double occupancy cabins
- full board (that's 3 meals a day, snacks etc)
- 4 dives per day
Land in Bali and Ujung: 1 nights in Bali, accommodation at Santika Bali or similar, breakfast, tax and service charge (no other meals included)
- 2 night at Manado UBUD , breakfast, tax and service charge (no other meals included)

International flight on EVA Airline:
